The training modelled the approach to be used in module delivery.Witnessing examples of how to run modules in practice, including theintegral group exercises, was considered to be a really useful trainingapproach by the participants.
However, a great deal of frustration was expressed about the time required toorganise CASCADE. In many sites complications around organising the modulestook up any time that site educators had allocated to practice andpreparation of the programme.
As site educators became more familiar with delivery and the group got toknow each other there was improvement in the fidelity techniquecomposite scores across the four modules. Early challenges were oftenovercome through increased confidence gained from running initialmodules and increased familiarity with content and less need to checkthe manual during delivery. Site educators said they had started to usethe approaches in every day clinical practice and felt better able todeal with silences and more able to facilitate discussion. Some saidthat when delivering a module they frequently referred to what aprevious group had said. This worked well as a technique to helpgenerate discussion in smaller quiet groups. Two sites started withrelatively high scores in the first two modules dropped off in modules 3and 4. These sites swapped site educators across the modules.
These implementation and model factors were compounded by trial factors,including the small numbers of recruits in some sites, an extended time lapsebetween training and delivery caused by delays in trial patient recruitment, andworkload related to supporting data collection. We would recommend that the siteeducators carry out some practice runs delivering modules before actual deliveryin the trial and be assessed for fidelity of delivery and constructive feedbackin any future such interventions.
